The following forms are for your convenience. Simply print, fill them out and fax, mail or bring them by the ABC ACADEMY center.

CHILD ENROLLMENT FORMS
In addition to the follwing forms, for any child who is not enrolled in public school
ABC ACADEMY will need a copy of the child's immunization record and a health statement from their doctor stating the child is clear to participate in a child care program. Once the completed forms are returned you will receive a copy of the Parent Handbook which includes a form that will require a signature.
For your convenience most of the forms included in the following packets are interactive fillable forms and can be filled in on your computer, saved, printed and emailed.
