ABC Academy Montgomery Childcare Enrollment

We understand how busy you are, so we have made enrollment convenient by providing all of our preschool, childcare, enrichment, and daycare enrollment documents in packets (see below). Simply print, fill them out and fax, email, mail or bring them by the ABC ACADEMY center.

CHILD ENROLLMENT FORMS PACKETS

In addition to the packet below, for any child who is not enrolled in public schoo,l ABC ACADEMY will need a copy of the child's immunization record and a health statement from their doctor stating the child is clear to participate in a child care program. Once the completed forms are returned you will receive a copy of the Parent Handbook which includes a form that will require a signature.
